We deeply regret to inform you that NorthStar Medical Group will be permanently closing its doors. It has been a gut-wrenching decision and please be assured that we looked at every available avenue to remain open but have been unable to find a way through. As such, we are not accepting new patients and will be wrapping up with our existing patients over the next 30 days as care is transferred to other medical practices.
If you need help finding another provider, please contact the North Carolina Medical Society or visit the North Carolina Medical Board website at http://www.ncmedicalboard.org for resources.
We are happy to transfer your records to the provider you designate. Since these records are confidential, we will need your written authorization to make them available to another provider. You can find the medical record transfer release authorization form on our patient portal of our website at northstar-med.com/patients.
Please complete the form and return it by January 23rd. If you would prefer that we email your records to you, we would be happy to do so, please email that request to us at email@example.com. However, be advised that email is not protected with encryption, so please acknowledge your understanding of that in your written request.
